Порівняльний аналіз впливу депресії на перебіг вагітності і пологів у підлітковому віці

Анотація

Актуальність. Депресія є одним з найбільш поширених ускладнень вагітності. Виражені депресивні розлади відчувають близько 12,7% вагітних жінок, і в 46% випадків даний стан спостерігається у вагітних підлітків.

Матеріал і методи. Проспективне, описове, селективне дослідження включило 168 (64,4%) вагітних. Вагітні жінки у віці 13-25 років, які пройшли психологічне анкетування в третьому триместрі вагітності і народили в пологовому будинку Центру Матері і Дитини в період з 01.01.2013 р. по 31.12. 2014 р.

Результати. Була встановлена висока частота депресивних станів у вагітних підлітків (OR = 2,42 (1,43 <OR <4,09), р <0,001), у порівнянні з жінками старшого репродуктивного віку,  що обумовлено обтяженим сімейним анамнезом, відсутністю підтримки з боку сім'ї чи соціуму, стресовими станами (травми, насильство, втрата, розлучення) (р <0,01). Також достовірно частіше у вагітних підлітків на тлі депресивних розладів зустрічаються ускладнення вагітності та пологів, такі як: блювота вагітних (OR = 3,9 (1,24 <OR <12.03), р <0,01), гестоз OR = 3,4 ( 0,9 <OR <12,8), (р <0,05), ЗВУР плода (OR = 2,43 (0,96 <OR <6,12), p <0,05).

Висновки. Результати проведеного дослідження підтверджують літературні дані про більшу частоту депресивних станів у вагітних підлітків, ніж у жінок старшого репродуктивного віку, обумовлену обтяженим сімейним анамнезом, відсутністю підтримки з боку сім'ї чи соціуму, стресовим станом (травми, насильство, втрата, розлучення). Також достовірно частіше у вагітних підлітків на тлі депресивних розладів зустрічаються ускладнення вагітності та пологів, блювота вагітних, гестоз, ЗВУР плоду. Таким чином, існує необхідність впровадження скринінгових програм депресії під час вагітності і надання допомоги вагітним жінкам, особливо з обтяженим з анамнезом щодо депресії.
